If would like to become a nextgen Preferred Vendor, please complete the registration form.
Once the initial information has been reviewed our Contracts Department will reply with registration packet for completion. (The Master Services Agreement is available for your review, please download.)
QUALIFICATION PROCESS
VENDOR
Vendor Questionnaire must be completed and approved
Completion of the W-9 Form
Certificate of Insurance
Vendor must sign Master Services Agreement which requires compliance to
Nextgen’s Customer flow down requirements
Submit current Diversity Certifications (if applicable)
1099 Consultant Process
Must comply with the IRS checklist
Provide Tax Identification
Provide Certificate of Insurance
